It has to be issued by an approved Aeromedical Examiner. Its cost depends on the pilot's medical history but it can be estimated to at least / around £150.<ref>Any experience?</ref>
Its validity period is 60 months until the license holder reaches the age of 40 (24 months if the license holder is between 40 and 50; or and 12 months if they have reached the of 50).
